Take away Don’t Do’s • Give guidance - Empower • Pediatrician help research available adult PCP • Collaboration with Pedi & adult PCP before and after transition • Adult PCP - hold a “Get to Know Our Practice” • Keep staff informed – have a “primary go to” person • Be accessible • Do not attempt to discuss this at age 18 • Do not divorce abruptly • Do not shop and drop • Do not dismiss any concern • Don’t hesitate to ask the families opinion • Don’t be afraid to form a relationship
