Integrify 5.0 Tutorial : Creating a New Form In this tutorial, we will show you how to: • Create a new form • Add different question types to a your form • Add and test field validations • Format your form by using the built-in Form Editor Need Help?   2. Adding a New Form Under the Administration section, select Forms, then Add New

  3. Adding a New Form • From the popup window: • Give the form a Name • Assign to a Category • Optionally add a Description Select Save

  4. Adding a New Form Once you have saved your form, you can edit the name, category and description by clicking on the fields for each You can also remove or copy a form here

  5. Adding Questions to a Form 1) Select Question List to start adding questions to your form 2) Select Label to type in the question name 3) Next, select Question Type to choose the type of question you want to add 4) Select Required to make it required or not 5) Select Add Question

  6. Adding Questions to a Form You can perform quick edits by simply clicking on the label, question type, required and ordinal fields if you want to make changes These indicators will allow you to perform further edits to your questions and copy or remove questions from your form

  7. Adding Questions to a Form If you select the Edit icon, you can make additional edits to your question such as: • Label Changes • Adding help for the questions • Including section headers • Defining the maximum field length • Assigning to a new page

  8. Adding Questions to a Form By selecting Database Lookup, you can query the internal database or an external database to prefill any question with data

  9. Adding Questions to a Form By selecting Custom Validation, you can add any custom field validation and the validating message for any field in your form

Adding Questions to a Form 1) If you are editing a multiple choice question, you will also have a Question Choices tab to define the choices 2) You can add the question choices here and optionally make any choice the question’s default value 3) If you want to edit your choice, just click on the choice name to perform your edit

Testing Form Validations 1) By selecting Form Preview, you can see how your form will display to users that are completing the form 2) You can test the field validations here (required fields or any custom validations that you have added) 3) Select Edit Layout to open the built in editor if you want to format your form differently

  12. Using the Form Editor Within the editor, you can change the appearance of your form. There are a number of different options for you to select from in the editor’s menu. You can do things such as adding images and tables to the form, changing fonts, adding color, inserting media or anything you want to do within a HTML form

  13. Using the Form Editor You can also get under the covers and make changes to your HTML source data by selecting HTML. For example, you could add javascript to make the form more dynamic if you want.

Using the Form Editor • You have several choices here: • You can select Restore Default to revert back to the default list view of all your questions in your form. • You can select Save Layout Changes to save the changes you have made to your form from with the editor mode • Select Default View to open a new window with all your questions. This allows you to simply drag and drop the questions into the editor layout. • You can always select Cancel Edit to cancel any changes to the form that were made in the editor.

  15. Using the Form Editor You can change the placement of fields with a drag and drop. Or if you want to add a new table with defined rows and columns, select the new table icon here. You’ll be able to drag questions into this new table.

  16. Using the Form Editor Another option is to select the Default View button at the bottom of the editor and drag any question to a position you want to drop it into.

  17. Using the Form Editor Once I selected Save Layout Changes in the editor, you can now see the updated you made to your form

  18. Locating Your Form You will always find your form by selecting View/Edit under the Forms section
