  10. District of Columbia • TheDelmarva Foundation of the District of Columbia, Inc.recruits Medicare beneficiaries and providers from: • 8 wards • 79 zip codes and • partners with 38 ancillary organizations

  11. Georgia • The Georgia Medical Care Foundation recruits Medicare beneficiaries and providers from: • 12 counties • 304 zip codes and • partners with 39 ancillary organizations

  12. Louisiana • The Louisiana Health Care Review, Inc. recruits Medicare beneficiaries and providers from: • 5 areas/parishes • 134 zip codes and • partners with 20 ancillary organizations

  13. Maryland • The Delmarva Foundation for Medical Care, Inc. recruits Medicare beneficiaries and providers from: • 2 counties • 136 zip codes and • partners with 22 ancillary organizations

  14. New York • IPRO of New York recruits Medicare beneficiaries and providers from: • 4 boroughs • 243 zip codes and • partners with 11 ancillary organizations

  15. U.S. Virgin Islands • The Virgin Islands Medical Institute, Inc. recruits Medicare beneficiaries and providers from: • 3 islands • 6 zip codes and • Partners with 16 ancillary organizations

  16. EDC Number of Targeted Practices and Patients in 9th SoW • A total of 579 physicians will be recruited. • A total of 10,977 patients will receive diabetes self-management education.

  17. EDC Partners EDC partners include, but are not limited to the following: • State Diabetes Prevention and Control Programs • Cooperative extension offices • Medical societies • Government agencies • National and state organizations • Community-based organizations • Faith-based organizations

  18. Preliminary Baseline rates for diabetes outcome measures for each state * In most cases, the disparity applies to the underserved population. An * indicates that non-underserved beneficiaries received the test less often than underserved beneficiaries.

  19. EDC Goal The overall goal of the Every Diabetic Counts Program is to reduce morbidity and mortality among underserved populations in the area of diabetes by educating Medicare beneficiaries on how to manage their disease.
