  8. Fit Note Fit note is the informal name for the new Statement of Fitness for Work. • Effective from 6th April 2010 • Employees may need to discuss possible changes at work with you: • Working different hours temporarily, • Doing different duties or tasks, or • Back pain – avoid heavy lifting
