  3. LPA Monitoring Survey To be completed by the Construction Monitor after inspections or other site visits Can be found online on the Division of Construction Management Homepage or the new LPA Construction Contract Administration webpage

Survey Results Each month the data is collected and analyzed There is an MS Excel Spreadsheet that is posted to the Construction Management Intranet Portal “Shared Documents” page Results are meant to reviewed by the Construction Monitors and DCE’s to help keep track of the LPA projects in their district that need monitoring http://portal.dot.state.oh.us/Divisions/Construction/Shared%20Documents/Forms/AllItems.aspx

  7. Possible Additions • For the Summary Page on the spreadsheet, we are looking to add the Encumberance Date • This will add another piece of information to assist in determining which projects are in need of monitoring
