Arrow Canadian Benefits Summary Review • Important Facts to Remember: • Healthcare coverage in Canada is mandated and offered to all residents by their respective government province • Employer plans are supplemental to the provincial coverage and do not stand alone • Married employees must cover their spouses unless thespouse is covered by another employer-sponsored plan

Manulife Extended Health Coverage Premiums paid By Arrow Medical • Annual Deductible – Individual $25 / Family $50 • Reimbursement - 100% • Hospital - Semi Private Room - 100% • Paramedical Coverage - $300/Practitioner/Year • Private Duty Nursing - $10K/calendar year, per condition • Vision Care – Exam included - Materials - $150 every 2 Years • Hearing Aids - $500 every 5 Years • Emergency Care Out of Province – 100% Coverage ends at age 70

Manulife Extended Health Coverage (continues) Premiums paid By Arrow Drug Card – Pharmacy Benefit • 100% after Dispensing Fee ($7 Max per Rx – plan pays) • Enhanced generic substitution program • $15,000 lifetime maximum on fertility drugs • $300 lifetime maximum on anti-smoking prescriptions drugs Coverage ends at age 70

Manulife Dental Benefits Premiums paid By Arrow • Annual Deductible - $25 individual/ $50 Family • Preventive/Basic Care – 100% • Major Restorative Care – 50% • Orthodontics – 50% (children up to age 19) • Plan Maximums – per member • $2K– Preventive/Basic/Major Combined per calendar year • $1K – Orthodontics Combined – per lifetime Coverage ends at age 70

Manulife Life Insurance Benefits Premiums paid By Arrow Basic Life Insurance* • Employee = 2x Annual Earnings to a max of $500,000 • Non-Evidence Max = $380,000 Basic AD&D* • Employee = 2x Annual Earnings to a max of $500,000 • Non-Evidence Max = $380,000 Dependent Life** • Spouse - $5,000 • Child - $2,500 * Benefit amount reduces by 50% at age 65 and ends at age 70 or retirement ** Coverage ends at age 65 or retirement

Manulife Life Insurance Benefits Premiums paid By Employees Optional Employee & Spouse Life Insurance • Employee - increments of $10K to a max of $500,000 • Spouse - increments of $10K to a max of $500,000 • Non-Evidence Max - All amounts subject to Evidence of Insurability Optional AD&D • Employee - increments of $10K to a max of $500,000 • Non-Evidence Max = $380,000 Coverage ends at age 65

Manulife Disability Insurance • Weekly Indemnity Plan (Short-term Disability) Premiums paid By Arrow • 66.7% of weekly wages to a max of $2,000 per week • Maximum Duration – 17 weeks • Non-Evidence maximum - $1,750 • Elimination Period – 0 days accident/ 0 days hospital/7 calendar days for illness • Benefit Paid is taxable • Long-term Disability Premiums paid By Employee • 66.7% of your first $2,500 of monthly earnings; plus 50% of the next $3,500 of monthly earnings; plus 40% of any excess amount to a max of $10,000 per month • Maximum Duration – to age 65 or earlier if able to return to work • Non-Evidence maximum - $7,800 • Elimination Period – 120 days • Benefit Paid is non-taxable Coverage ends at age 65

Benefits Enrollment Process • As a new employee, you must complete the following forms located on the Arrow Benefits Website • http://benefits.arrow.com/CanadianBenefits/HealthandWelfare/index.htm • Manulife Enrollment Form • Manulife Beneficiary Designation Form (original mailed copy required) • Optional Life Insurance Application and EOI (optional) • The Manulife Enrollment Form must be submitted to the HR Service Center and can be submitted via scan/email to hrservices@arrow.com, or via fax to (303) 704-4657.  • The Manulife Beneficiary Designation Form can be completed at any time, and the original form must be mailed to:  • Arrow Electronics, Inc. Benefits Enrollment Process (continue) Manulife ID Cards It can take up to one month for Manulife to mail out ID cards.  As soon as your enrollment has been processed with Manulife, you can register through Manulife's website and print off temporary ID cards if you need to use the coverage prior to receiving your ID cards. Registered Retirement Savings Plan/Deferred Profit Sharing Plan (RRSP/DPSP) • Eligible to participate the first of the month following one month of employment • May defer a minimum of 1% of annual earnings up to an annual limit established by the Canada Revenue Agency (CRA) • All Savings and investment earnings are tax deferred until the time of your distribution • Arrow will match 50% of savings up to the first 6% of pay deferred • Vest in employee RRSP immediately, vest in employer DPSP after 2 years of plan participation
