You are in the process of enrolling in the Home and Community-Based Services Program (HCS) or you are looking for a new HCS provider. One of the first decisions you will be making is choosing your provider! It is very overwhelming to choose from the list of providers you were given in your area. Although the task of choosing a provider can seem overwhelming—don’t let it be. Choice allows you to make a good match for you and your family.

Every provider is required to offer all and the SAME HCS services. But how they provide the service may differ. At PTCC WE ENCOURAGE YOU TO educate yourself about the services and supports available through HCS.

At PTCC all staff undergoes a criminal history background check, competency training, personality screening and more! Ask about how you will be involved in staff selection. We have pulled together ALL our resources learned from working in various State Departments in TEXAS, TENNESSE and MISSOURI to develop and unique approach to service delivery and community integration.

Ask how problems or concerns will be addressed. Was the provider respectful to you during the interview — did the provider use terms that you did not understand, did the provider maintain eye contact and listen to you or did they do all the talking? Ask about the complaint process. Ask how they work to improve services. What overall feeling did you have during the interview— did you feel comfortable? PepperTree Complete Care“Where Compassion Meets the Heart” Community Living Assistance and Support Services (CLASS) CLASS provide home and community based services to people with disabilities or a related condition as a cost-effective alternative to placement in an intermediate care facility. A related condition is a disability with a primary condition other than mental retardation that affects the ability to function in daily life. CLASS is a Medicaid waiver program providing individualized services in the person’s home and is administered by the Department of Aging and Disability Services (DADS). PTCC provides Case Management Services for the CLASS program in Houston, Texas with regional offices in Austin, Beaumont, San Antonio, Fort Worth, and Lufkin

HCS is a Medicaid waiver program providing individualized services in the person’s home and is administered by the Department of Aging and Disability Services (DADS). PTCC provides HCS Services in Houston, Texas with regional offices in Austin, Beaumont, San Antonio, Fort Worth, The Valley and Lufkin Catchment areas.

PepperTree Complete Care“Where Compassion Meets the Heart” General Revenue Services General Revenue Services is commonly referred to as “GR.” These services are offered to families on waiting list to any wavier program. All services are based on individual needs and must be identified by an interdisciplinary team. Services and supports to eligible persons as an alternative to the Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR) program. PTCC provides GR Services through the Gulf Coast Center Local Authorities.
