Eligibility Criteria • Inmates identified by jail administration or public health nurses or by inmate • Diagnosis of serious and persistent mental illness • Significant social or health concerns/needs • Identified CD/drug dependency issues • Homeless upon release • Willing to participate in RAP • Stearns County resident/releasing to Stearns residence

27. RAP Process • Initial meeting with inmate • Jail Administration or • PHN in jail • Short questionnaire • Determines eligibility • Preliminary direction

28. RAP Process • If inmate interested in participation • Consents-releases obtained • Schedule on RAP agenda • Staff notified of RAP participants • Bring pertinent information to RAP • PHN meets with Inmate to do more in-depth assessment.
