Performance Delivery – overall summary (1/4) Supporting Measures Headline Measures • The achievement is based on the current months performance (although this may relate to a previous month due to the availability of data) • Where a table indicates progress, the progress relates to the most recent published data where available or monthly or weekly leading measures for each indicator. Additional Public Health Measures • Public health measures- change in indicator- breast screening uptake for 50-70 yrs is now being measured instead of coverage. 2010-11 and 2011-12 performance has been amended to reflect this. CVD Mortality and Cancer Mortality- no longer using a 3 year rolling average as actual but instead using the annual rate. • Note: Cluster have re-issued RAG thresholds which may impact on how some of the targets are reflected in Cluster documentation. The new thresholds will be used in the Board reports for 12/13 reporting. 3

  11. 2011/12 Performance Delivery - Summary (6)GP Consortia A summary of GP Consortia performance is noted below however please refer to appendix 2 for the more detailed performance dashboards by Consortia • Immunisations (Q3): • Kingsbury met all 6 immunisation indicators. • Harness met 4 out of 6 indicators • Willesden met 2 out of 6 indicators • Kilburn and Wembley did not achieve any of the indicators • Smoking quits: • No localities have met their YTD Dec plan for smoking quitters • Health Checks • Achieved annual target for Health checks programme which was set at 16632 invites & 8316 completed checks • Currently we are at 24770 invites and 8773 completed Health Checks ( As at end of Jan 2012) • Breastfeeding 6-8 weeks (Q3): • Harness, Kilburn and Kingsbury met the 95% coverage target. • Harness, Kilburn, Kingsbury and Wembley met the prevalence target of 72.7%. • Breast screening coverage: • Kingsbury and Willesden achieved the 70% target for women aged 53-70 yrs screened for breast cancer in the last 3 years as at 30th June 2011. (based on provisional Q1 2011-12 KC63 data) • Kingsbury and Wembley were above the PCT % of 32% of women aged 47-73 yrs invited for breast screening in the last 12 months as at August 2011 • Cervical Screening: • Coverage as at Q3 2011-12- all 5 clusters met their 2% and 4% above baseline incentive target for women aged 25-49 yrs. Wembley met the national target of 80% for women aged 50-64 yrs.

  12. 2011/12 Performance Delivery - Summary (7)Social Marketing/Health Promotion Breastfeeding • 19th January 2012 - Breastfeeding event in Brent Town Hall promoting breastfeeding and other priority areas – over 120 mothers and health professionals attended and an evaluation report is available • In addition to 13 children centres, Brent Breastfeeding Peer Support Workers have started working in Northwick Park Hospital in partnership with the Harrow Breastfeeding Peer Support team • The Breastfeeding team aim to develop breastfeeding cafes in Brent. NHS Health Checks • NHS Health Checks promoted in Brent magazine (Jan edition) • NHS Health Checks on Life Channel (Jan and Feb) through GP surgeries in Brent • Leaflets and posters will be distributed to GP surgeries in Feb Cervical screening • 31 March 2012 – Elder Care Event (Venue to be confirmed) • January-February: A health promotion specialist in place to promote cervical screening in younger women Immunisations • 13 nurseries and playgroups have taken advantage of the early years small grant to promote immunisations e.g. Kensal Green under Fives held a parents evening on 29 Feb, Happy Child Ltd will organise mini campaigns and twilight sessions at Kilburn and Harlesden nurseries in March and April Smoking, Immunisations, NHS Health Checks • Social marketing events covering all the above took place in December at Brent Health Partnership Day, Kilburn Square and at Freuchen Medical Centre • Planned PH Partnership events: Harlesden 15th February 2012, Kingsbury 2nd March Smoking Cessation • Red Fun February British Heart Foundation Partnership Event – Wembley Central Square (159 referrals) • Stop Smoking Day on 14th March - Stop Smoking campaigns will be held at Brent locations including Brent Town Hall, Wembley Station, Kilburn Station, Queen’s Park Police Station and Wembley Police Station • 9 GP practice recruitment campaigns for Q4 - Completed • 8 Pharmacy mini campaigns completed
