  2. Parent Consent/Attempts

  3. How many attempts do you have to make to satisfy ODE that you attempted to get parent input? • Attempts for Three different dates. • Three Attempts for a single date. • Three written attempts

  4. When doing an FBA – do you need Parent Consent (PR-05)? • Only if it is due to a manifestation determination • Always • Never

  5. When do you need written consent ? • Only on the first IEP and whenever a change in placement is proposed. • On the first IEP and every IEP afterwards, including change of placement. • Only on the first IEP.

  6. When you are getting parent consent for new/additional testing, it must be… • In writing • Verbally or in writing • On a PR05

  8. When developing PR-01’s…. • You can have a standard set ready to go ahead of time • You can develop it at the meeting or after the meeting. • You must develop it after the meeting.

What must be included in a PR01? • Answer the questions • Answer the questions and be sure to include parents input • Answer the questions, include parents input and the discussion that took place- your meeting notes. • Answer the questions and attach your meeting notes for the entire dialogue.

  11. If you are going to do an ETR on the same day as the IEP and combine meetings can you combine PR-01’s? • Yes • No

  12. When adding a related service to an existing IEP you…. • Get a PR05 for the new assessment, and amend the ETR and IEP. The ETR dates will stay with the original. • Do a totally new ETR. • No data or assessment is needed, just add the service on the IEP.

  13. If you get a move in…. • You can accept an IEP but not the ETR. • You must have an existing ETR that is accepted prior to having an IEP.

  15. For a preschooler who qualifies under speech … • A continuum of services must be offered (itinerant > center based) • You must have itinerant teacher services. • You would always only have SLP services.

  16. An Initial IEP be implemented if… • The parent didn’t attend, but gave verbal consent over the phone • The parent didn’t attend, but 3 attempts were documented • The parent gives written consent on the IEP.

Who needs a written excusal if they can’t come to the meeting • The magic 4 (regular ed, special ed, district rep, parent) if they don’t attend the entire time. • The magic 4 – if they don’t attend even part of the meeting. • The magic 4, plus any other related service provider (attached to a goal)

  18. For a Preschooler – • You don’t need a regular educator as part of the IEP team. • You do need a regular educator as part of a team. • You need a regular educator as part of a team but a team member can hold 2 roles.

  19. At the conclusion of the IEP, if everyone is in agreement…. • You have to send a PR-01 at the conclusion indicating a new IEP was completed and all are in agreement • You have the parent sign – which meets the PWN requirement. • You send a PR01 indicating that you are proposing to change the IEP, prior to the IEP.

For an upcoming IEP meeting you would… • Send the PR-01 for the proposed changes in the IEP after the IEP • Send the PR-01 for proposed changes in the IEP prior to the IEP meeting with the invitation (PR-02) • Send the PR-01 for proposed changes after the IEP and the IEP can not be implemented until the parent has received this.

  21. Who needs to have written excusals if they can not attend the IEP? • Only the magic 4 who can not attend in whole or part. • Any team member (including related services) who have a goal on the IEP and can not attend. • Only the magic 4 who can not attend.

  22. What goes under Support for School Personnel? • Consultation between adults • Aide services for the student • Para pro support for pre teaching vocabulary for a lesson • All of the above

  23. In Best Practice- Once a student has gone through an FBA and had a BIP created…. • You just document your reviews on the BIP form, until a new IEP is created. • You just document your reviews on a BIP. • No documentation is needed.

  24. If in the IEP under special factors Behavior is checked as a need… • You must have a goal • You don’t always have to have a goal, or accommodation • You must have a goal or accommodation.

  25. Amendments to the IEP

  26. Who must be present for an amended IEP? • It does not require a team meeting. Anyone can contact the parent and discuss the change. However it should be a team decision • The entire team must be present to give feedback. • The magic 4

  27. In Best Practice…If an amendment is made… • You must document that all team members are aware of the changes that impact their provision of services. • You must rewrite the IEP. • You must document that parents and district representative are in agreement.

  28. What can be amended on the IEP? • Only minor changes. • Any change that all agrees to. • Any changes except adding a new goal.
