Checkpoint 1 Introduction Why is transition planning vital to student success in high school? • Louisiana Graduation Rate - 70.9% graduate • Lafourche Graduation Rate - 78.9% graduate • 2011-----SLHS----83.1 • 2011-----THS-----78.2 • 2011-----CLHS----76.2 LDOE…Cohort Graduation Rate 80% by 2014

  4. Checkpoint 2Second Time Frame • LPSB has established a new protocol for 8th grade IEPs. • When an IEP is held during the school year (8th), the IEP must address FAPE for an entire IEP year which includes a second time frame. • This will reflect the minutes at each high school.

Checkpoint 3 Transition Plans • LPSB has established a new protocol for 8th grade as it pertains to transition plans. • When IEPs are written throughout the year, all 8th graders must have a complete transition plan. • Prior to IEP meeting… • Graduation plan obtained from Counselor prior to IEP meeting signed by student & parent. • Complete teacher, parent, student transition questionnaires • Obtain Explore Test Results or complete career clusters and/or interest inventory • For the transition plan goals… • “After graduating from high school, I will…”

Checkpoint 4 Closing Out / Adding Goals • As a result of Monthly Monitoring at the High Schools, it was noted that some middle school students achieved their IEP goals before the IEP expired. • The middle school teacher noted that the student achieved his/her and closed out the progress report without adding another goal. • Consequently, students started high school without any goals.

Checkpoint 5 New Procedures for Achieving Goals • If a student achieved a goal, the middle school teacher must add another goal. • The IEP is in place for an entire IEP year; therefore, the student should have goals for the entire IEP year (Goals should be projected for the IEP year, not a two or three week period). Round About… Keep the Goals Going for one IEP year.

  8. Checkpoint 6New Procedures for Transferring IEP Folders • LPSB has established a new protocol for transferring IEP folders from middle schools to high schools. • Middle School Dept. Heads will transfer all 8th grade IEP folders including Connections to all high schools between May 25-28, 2012. • The liaison assigned to your school will provide you with the transfer of folders form to be signed by the middle school dept. head and an administrator from the high school. Checkpoint 7Collaboration with High School Teachers • When scheduling/writing IEPs for 8th grade students, the teacher of IEP authority will contact the high school teacher by email or phone to collaborate on writing transition plans and establishing the 2nd time frame. • The high school teacher will be invited to attend the IEP meeting. • Confirm that the high school teacher will attend before putting that teacher’s name on the IEP. • Refer to the End of School Protocol for specific deadlines. Take time to collaborate with the high school teachers
