  2. How to be successful in OJT • PAPERWORK+ DEADLINE MET= A • Be at school • Go to work • Keep your job, all year

  3. Attendance • Will have to meet with me during one of your designated OJT period(s), at least 1 time per month. • You and I will schedule those “consultation”

  4. OJT Credits • 1 Class= 10 hours per week • 2 Classes= 15 hours per week • 3 Classes= 20 hours per week

  5. What do you need? • Legal employment: • Paying taxes. • No Hazardous jobs. • Cannot work during your non-OJT class time. • Cannot have a family member as a direct supervisor. • Cannot work for a “family business.”

  7. Training Agreement • Fill out the left side (your information) • Supervisor fills out right side. • Insurance must be filled out and completed • Sign and date

  8. Training Plan • Fill out the front sheet • Need all signatures • Even if you are 18 you must have parent/guardian sign.

  9. Time Card • Fill out when you work • Summative portion of your grade • Every day late = Letter grade reduction • If you don’t submit one, you are removed from OJT.

  10. Report Card • Supervisor must fill out • Supervisor must sign • Summative grade • Every day late = letter grade reduction • No submission within week, removal from program.

  11. Insurance & Emergency Information • Please fill out as much as possible • Need signatures

  12. Grading Information 60/40 • Summative Grades: 60% • Time Cards (monthly) • Report Cards

  13. Grades Continued • Formatives: 40% • Visitations (1 per 9 weeks) • Training Plan Check • Pay Stubs • Training Consultation (1 per 9 weeks, information on next slide)

  14. New this year 2013-2014 • Training Consultation • Will review your current grades at University High School.* • Review due dates for paperwork • Set worksite visitations *If you are falling under 2.0 GPA for current grading period, you might have to come back for more consultations until grading improves.

  17. Missing work… • Must fill out a request for absence form, must be signed by your boss and approved by me BEFORE you miss work. • This is only if you might fall short for your hours that week.
