Assessment and improvement of safety culture at the OL3 nuclear power plant construction site: An additional inspection in 2008and follow-up inspections in 2009 and 2010 Examples of inspection questions for interviewing employees and supervisors at the OL3 site: 1) Understanding of safety significance, risks and quality requirements: • Can you briefly describe the quality requirements of your job? • Why do these requirements exist? • What is the safety class of your job? • How does the quality of your work affect the quality of this structure? 2) Following the instructions: • What instructions are related to your job? • Can you show us the corresponding instructions and drawings? • Are the instructions and drawings clear? • How do you utilise these instructions in your ongoing job?

Assessment and improvement of safety culture at the OL3 nuclear power plant construction site: Examples of inspection questions for interviewing employees and supervisors at the OL3 site 3) Safety culture training • What kind of training you have received from the vendor / the licensee? • Is the term “safety culture” familiar to you? How does it relate to nuclear safety? 4) Identification and handling of non-conformances: • If you notice problems in your job, how do you act? Whom do you report to? What would be an example of a problem that you should report? • Can you give as an example of a problem that you have encountered and reported? Describe the process and how your company reacted. 5) Atmosphere: • Can you freely discuss any problems facing your job at the site (with colleagues, the vendor, the licensee, STUK)?

OL3 Lessons learnt: safety culture • The concept of safety culture is challenging to communicate and implement • Examples of some key issues to be essential to communicate: • nuclear safety culture <=> occupational safety • nuclear safety culture => quality requirements • nuclear safety culture => requirements are met by following instructions • nuclear safety culture => competences and training • nuclear safety culture => systematic handling of non-conformances • nuclear safety culture => an organisational culture where (nuclear) safety is put first in all actions and decisions • Licensee has to communicate quality and safety requirements and survey that these requirements are met • IAEA Guide GS-G-3.5 (2009) gives support to develop and assess the safety culture
