Minimum Eligibility Requirements • Applications will be accepted from the following not-for profit organizations: • Designated AIDS Centers • Academic Medical Centers • Special Needs Plans (SNPs) • *Community-based & professional organizations may apply for Center F only (Resource, Referral and Evaluation) • Applicants must be located in New York State.

General Program Requirements • Staff experience and skill providing HIV clinical education • Ability to integrate clinical education priorities and AI HIV Guidelines • Organizational structure • Ability to form collaborative relationships with other organizations • Participation in CEI program meetings • Quality Management Program • Program and fiscal reporting to the AIDS Institute

Preferred Eligibility Requirements Center A: Clin Ed for Upstate Providers • Experience providing HIV clinical care and clinical education • Organizational capacity and resources to plan, develop and implement HIV clinical education throughout upstate NY • **Applicants must be located in one of the upstate counties listed in Attachment 10

Preferred Eligibility Requirements Center B: PEP, Testing & Diagnosis • Experience providing HIV clinical care and clinical education • Organizational capacity and resources to plan, develop and implement HIV clinical education throughout NYS • Experience providing education and consultation about PEP for adults, adolescents and children • Expertise in HIV testing technology

Preferred Eligibility Requirements Center C: Prevention & Substance Use • Experience providing HIV clinical care and clinical education • Organizational capacity and resources to plan, develop and implement HIV clinical education throughout NYS • Experience providing HIV clinical education about preventionand substance abuse

Preferred Eligibility Requirements Center D: Mental Health • Experience providing HIV clinical care and clinical education • Organizational capacity and resources to plan, develop and implement HIV clinical education throughout NYS • Established HIV mental health education and training program

  11. Preferred Eligibility Requirements Center E: Technology • Technical expertise and capacity to develop a multi-media, web-based educational program

  12. Preferred Eligibility Requirements Center F: Resource, Referral, Evaluation • No Preferred Eligibility Requirements • See Minimum Eligibility Requirements: Non-profit DAC, academic medical center, SNP, community-based organization or professional organization

Center A: Clin Ed for Upstate Providers • Award Amount: $400,000 • Target Audience: • Frontline HIV primary care providers - outpatient • AI-funded health centers that have received continuous CEI training and education • Upstate counties listed in Attachment 10, with the exception of Westchester and Rockland counties • Members of the clinical team: physicians, PAs, NPs, RNs, dentists, pharmacists

  36. Center F: Resource, Referral, Evaluation • Referrals: • Solicit and manage requests from community-based providers • Assist in developing an agency-based education plan for staff • Resource Guide: • Develop and AIDS Clinical education and training resource guide for providers

Resource, Referral, Evaluation (cont’d) • CEI Program Evaluation: • Development of the evaluation methodology to assess outcomes and impact of clinical education provided by CEI • Standardize data collection instruments, develop protocols for data submission, quality assurance procedures, report on aggregate data • Update existing CEI reporting database

Completing the Application • General Format: • Application should not exceed twenty-five (25) double-spaced pages • Page limit does not include budget, budget justification, and all attachments • 12-pitch type font • One-inch margins on all sides • **Non-compliance with format requirements may result in the deduction of up to 5 points

Review Process • Applications will be reviewed and evaluated using an objective rating system reflective of the required items specified for each section. • A panel convened by the AIDS Institute will conduct a review of applications from eligible applicants. • The review team will base its scoring on the maximum points indicated for each section. • Application with the highest acceptable score for each Center will receive the award.

Review Process • Reviewers will consider the following factors: • Clarity of applications • Responsiveness to the RFA • Agency capacity • Demonstration of sound training plan • Applicants access to target audience • Comprehensiveness of program design • Appropriateness of evaluation strategy • Justification for costs in budget
