  3. Research Networks • The National Institute Health Research (NIHR) Clinical Research Network (CRN) is working to develop and strengthen NHS infrastructure to support the delivery of clinical research 

  4. Research Networks • Clinical Research Networks- coordinate and support research in: • Cancer (CRN) • Dementias and Neurodegenerative Diseases (DeNDRoN) • Diabetes (DRN) • Medicines for Children (MCRN) • Mental Health (MHRN) • Primary Care (PCRN) • Stroke (SRN)

  5. Research Networks • A Comprehensive Clinical Research Network supports research in all areas of disease and clinical need. • 25 Comprehensive Local Research Networks (CLRNs) across England, including 2 South West CLRNs: • PenCLRN • Western CLRN

  7. How the networks can practically support Chief Investigators of single and multi-centre research studies

Chief Investigators and their teams Access to a whole network of sites and information Detailed Feasibility including: Patients available e.g. past experience / over researched Capacity of sites Facilities Strategic site identification and recruitment strategy Promotion of the study Clinical / non-clinical advice on practicalities of the protocol design Support to ‘set up’ study across a number of sites Support to ensure sites are able to undertake the study e.g. generic training / staff development Assist with performance management / ongoing issues

The ESTEEM trial: Triage Systems for Same Day Appointments in General Practice (£2.5m) multi-centre trial (Intensive intervention in each practice Triage - Nurse, GP or usual care) PCRN Director advised on protocol Approved for funding by HTA Linking early with CI in order to facilitate adoption process and agree recruitment strategy Advice on Service Support Costs and Excess Treatment Costs Close links with the three academic Departments of Primary Care Chief Investigators and their teams

Principal Investigators and Research Teams • Support staff in all aspects of research to aid recruitment • Study feasibility & review • Ensure Service Support Costs & RM&G approvals obtained • Assist with recruitment strategy • Level of staff support agreed • Promotion of studies through Primary Care Incentive Scheme • access to over 1,000 general practices, 20-25% have agreed to do NIHR PCRN studies • Information about forthcoming studies (email alert to site/practices) • Training and education in generic research activities (GCP) • Sharing of solutions and ideas relating to the study across sites/practices

Whole System Demonstrator Programme (WSD) (PCRN & CLRN) Cluster randomised trial (telemedicine – chronic disease) Protocol reviewed by PCRN Meeting research team early to agree level of support Recruitment in an under researched geographical area Research Officer provided to identify patients & facilitate recruitment – (PCRN/PenCLRN) Rates of Psychiatric Illness in Long-term Survivors of Early Childhood Brain Tumours (Psych-BTS) (PCRN & MHRN) Rare condition Recruitment strategy agreed across South West (PCRN/MHRN) Teleconference involving all Networks Plan formulated to ensure Research Officers cover across 13 PCTs Training requirements agreed

Principal Investigators and Research Teams Example: Increase in Topic Network Research In North Devon CLRN/DRN/SRN Working with the comprehensive network to set up a new joint research nurse post in North Devon. This has enabled the increase of the diabetes and Stroke research portfolio and new links with the consultants and clinical teams In general the networks have supported new investigators in both primary and secondary care to begin to build a local portfolio and integrate research into their clinical service thus increasing NHS infrastructure

South West (SW) Network Managers Group • Representation from CLRNs, PCRN (SW), TCRNs which have a presence in the SW • Meets quarterly • Discuss key work areas which networks have a remit for to ensure integrated SW approach including: • Training and Education • Patient and Public Involvement • Industry

  18. Streamlining funding application processes • Streamlined system for applying for FSF funding to ensure no duplication • SW application forms and guidance

Working together in primary care • Primary Care Research Network (PCRN) South West (SW) • Covers 3 CLRNs (including PenCLRN and Western CLRN) • Joint CLRN/ PCRN Research Officers/ Clinical Lead posts • Joint CLRN/ PCRN Clinical Study Officer post • Endorsement of the SW PCRN costing template • Involvement in the Primary Care Incentive Scheme (PCIS) • Co-ordinated approach to funding sessional support in primary care

Study Example (1): Working together to help deliver research • COCOA:Care of Offenders: Continuity of Access- UKCRN ID 5131 • UK Multi-centre observational study funded by NIHR Service Delivery and Organisation Program • NIHR Portfolio Study- co-adopted by PCRN • Chief Investigator based in Plymouth with research sites across PenCLRN and Western CLRN • Research team struggling to recruit into study so approached Pen CLRN for support • Teleconference with mental health trust in the area, PenCLRN SW MHRN and PCRN • Arranged for Clinical Studies Officers to help recruitment in all sites including Plymouth, Exeter and Bristol

Study Example (2): Working together to help deliver research • MASCOT: Management of Asthma in School-age Children On Therapy: UKCRN ID 3774 • UK Multi-centre interventional study funded by HTA • NIHR portfolio study- adopted by MCRN and co-adopted by PCRN and respiratory • Chief Investigator based in University Hospital of North Staffordshire • WCLRN and PenCLRN involvement via PCRN (SW) • PCRN (SW) staff are training Practice staff to take part in the study, supporting Practice database searches and are preparing recruitment material to be sent by Practices

Summary • SW networks aim to increase the number of studies available on the Portfolio and maximise on recruitment • SW networks are committed to ensure any potential ‘blocks’ to research are resolved • CLRNs, PCRN & TCRNs are committed to providing the necessary support to ensure the SW is fully supported to participate in NIHR portfolio studies
