  1. 大區域骨損傷部位再生之新策略 織工程的三大原則:細胞、間質(matrix) 和組織生長分化因子,其中生長分化因子,居誘導和調控的角色,對細胞的分化、間質之形成有決定性的影響,哈佛大學醫學院血液研究中心主任Dr. Kevy 與 University of Miami, 口腔外科主任 Dr. Marx 多年合作【2,3,9】,發現如以一定劑量的 thrombin 來激發血小板,使之釋出growth factors,如PDGF,TGF-ß,VEGF,EGF等, 會誘導mesenchymal stem cells, endothelial cells及其他細胞發生mitosis , replication 和 differentiation,促進骨組織再生與創傷之癒合,同時亦發現血小板被激化後,所釋出的生長因子量和血小板量是成正比關係,亦即血小板的數目越高,則釋出的生長分化因子也相對越多,從此開啟了利用platelet-rich plasma (簡稱PRP,又稱為platelet concentrate即含有大量血小板的血槳),來加速骨組織再生與軟組織癒合的研究,因為platelet concentrate來自病人本身,故又稱為 autologous platelet concentrate ( APC)。因PRP血槳內蘊含相當量的 fibrinogen,和thrombin 作用後,會形成 gel 狀之 fibrin-rich matrix,此matrix 不但可以包覆、攜帶骨移植材(bone graft materials),且可提昇接著性與可塑性,不但有助於創傷的癒合,而且非常適合作為誘導骨組織形成之 scaffold(骨架),對於未來臨床的應用有相當大的裨益。
