Excavator Operator’s Declaration Form • On arrival at site, and as part of the site induction, excavator operators are required to complete and sign a declaration to the effect that they have had the necessary training for the specific equipment that they will be using on site, including any non standard equipment such as: safe load indicators, quick hitch mechanisms, pile hammers / extractors, pole grabs and breakers in addition to standard excavation duties.

  3. Excavator quick hitch attachment register • The register is completed for every attachment change and includes; 360, 180 degree excavators and mini diggers. • Where there is no attachment change in the course of the day, the register is signed on a daily basis to confirm that the driver has inspected the mechanism and that it is in good order. • A key for the attachments is provided on the register for identification purposes. • The person who is identified as having responsibility for plant on site will inspect the register at a maximum of seven days to ensure that the form is being used correctly.
