  3. HBV must be completed before entering the program • Eligibility issues will require a Petition of Declaratory Order • Go to: http://bon.texas.gov. Click on forms. Scroll down to Declaratory order and follow instructions. • http://bon.texas.gov/olv/pdfs/DOapp.pdf

  4. Website: • May contact: Texas Board of Nursing with questions • Criminal background checks are performed on all applicants for Licensure in Texas. A criminal background check must • be completed through the Department of Public Safety (DPS) and the Federal Bureau of Investigation (FBI) based on • the set of fingerprints you provide to MorphoTrust. The BON cannot accept fingerprint cards or criminal • background check results mailed by the applicant, or results that were completed for another facility, even if • the previous check was completed through DPS and FBI.

  5. Criminal Background Check • A person convicted of a felony /misdemeanor of a court of competent jurisdiction enters an adjudication of guilty against the person on a felony offense under the laws of this or another state of the United States. • These crimes which the Board considers to be directly related to the duties and responsibilities of a Licensed Vocational Nurse shall include, but are not limited to: • Offenses against a person • Offenses against property • Offenses involving fraud, dishonesty or deceit • Offenses related to drugs/alcohol • Offenses which include attempting or conspiring to commit any of the offenses in this subsection

  6. Admission Criteria *****Beginning fall 2014 the TEAS test must be taken and passed before enrolling in prerequisite courses VNSG 1320 and VNSG 1216 will be required for program acceptance.

  7. Essential Functions • Essential functions are the basic physical, cognitive, psychomotor, social and affective activities that are necessary for successful completion of the Houston Community College Vocational Nursing Program curriculum leading to initial licensure as a vocational nurse. • Basic functions included are gross motor skills, fine motor skills, physical endurance, physical strength, mobility, hearing, visual, tactile, smell, reading, arithmetic competence, emotion stability, analytical thinking, critical thinking, interpersonal skills , and communication
