HSAG of California: Improving Dementia Care What has your organization been doing to improve dementia care? • Hosting learning and action network (LAN) sessions • Conducting nursing home on-site visits • Creating an antipsychotic medication reduction toolkit • Working with the California Association of Health Facilities (CAHF) • Conducting Webinars

HSAG of California: Improving Dementia Care (cont’d) What do you see as the priorities for moving forward? • Continued Quality Assurance & Performance Improvement (QAPI) education for nursing homes. • Nursing home education on person-centered care approaches to understand “needs-driven expressions” in residents with dementia. • Improvement in care transitions across the care continuum related to caring for individuals with dementia.

HSAG of California Improving Dementia Care (cont’d) What do you hope to accomplish through this partnership? • Continued statewide support with improving dementia care and reducing of off-label use of antipsychotics through education, training, and engagement of key stakeholders and nursing homes.
