  1. Nebulized Drug Administration

  2. Conditions The candidate should perform this skill on a simulated patient in a sitting or supine position (stretcher, chair or bed) under existing indoor, ambulance, or outdoor lighting, temperature, and weather conditions.

  3. Indications A patient whose condition requires the administration of a medication through the nebulized route

  4. Red Flags Equipment used to nebulize medications can vary significantly. Practice with your local system’s equipment until you are comfortable with assembly and use.

  5. PPE!

  6. Assess level of respiratory distress

  7. Explain procedure to patient while preparing equipment

  8. Inquire about medications and allergies

  9. Right Patient Right Drug Right Dose Right Route Right Time Five Patient “Rights”

  10. Place medication in nebulizing chamber

  11. Screw on chamber cover

  12. Attach oxygen tubing to nebulizer chamber

  13. Attach tubing to oxygen source

  14. Assemble delivery system according to manufacturer’s recommendations

  15. Ensure nebulizer chamber remains upright

  16. Attach T-piece to nebulizing chamber

  17. Adjust the flow of oxygen to nebulizer to create a steady mist of medication (6-8 LPM)

  18. Direct patient to breathe deeply from mouthpiece

  19. Secret: Direct patient to count, “2 seconds in, 2 seconds out”

  20. Monitor for desired and adverse effects

  21. Refill chamber per local protocol

  22. Inline Nebulizer

  23. Attach inline setup to nebulizer chamber

  24. Chamber connects to T-piece

  25. Adjust the flow of oxygen to nebulizer to create a steady mist of medication (6-8 LPM)

  26. Ventilate at 12-20 BPM timed with patient’s inspiratory effort

  27. Document dose and time of administration and monitor for desired and adverse effects

  28. Use PPE!!!

  30. Select proper equipment

  31. Keep nebulizer chamber upright

  32. Assist ventilations as needed

  33. ALTERNATE METHOD:Mask Nebulizer

  34. Attach nebulizer to simple mask

  35. Can also use NRB mask with reservoir and tab removed

  36. Follow same breathing rules as with T-piece
