  1. INCIDENT During lay down 8 ½” bottom hole assembly,  the drilling crew picked up the 6 ½” Jar from the mouse hole by using a hydraulic winch operated by Derrickman. Once the jar was out of the mouse hole, IP guided the jar in position with his left hand to the V-door while his right hand was holding the handrails. In the process of lowering the jar into V door, the Jar bounced / swung and hit the IP right hand finger. ROOT CAUSES: Sudden/fast movement of the winch Incorrect Hand position “Jar hit the IP’s right hand” finger Technical design constraint (winch & cathead linked in one hydraulic pump/manifold line, interdependency caused loss of control during operation), Inadequate hazard identification & controls Lateral Learning Ensure Rig Safe Operating Procedure adequately covers hazards of interdependency during simultaneous operations Keep away from potential pinch points. Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction keep away from potential pinch point.
