Thevastmajorityofbackproblemscanberemediedwithnon-surgicaltreatments—oftenreferredtoasnon-surgicalorconservativetherapies. Ifyou’vestruggledwithbackpainforanylengthoftime,youmaybewonderingifspinesurgeryisyouronlytreatmentoption.Sometimes,surgeryistheonlytreatment.However,there’sgoodnews.Thevastmajorityofbackproblemscanberemediedwithnon-surgicaltreatments—oftenreferredtoasnon-surgicalorcon-servativetherapies.

CONSERVATIVETREATMENT Aswithallnon-emergencyspinalsurgeries,atrialofnon-operativetreatment,suchasphysicaltherapy,painmedication—preferablyananti-inflammatory, or bracing should be observed before surgery is considered.Thetrialperiodofconservativetreatmentvaries,butsixweekstosixmonthsisthegeneraltimeframe. Spinesurgerymayberecommendedifnon-surgicaltreatmentsuchasmedicationsandphysicaltherapyfailstorelievesymptoms. Surgeryisonlyconsideredincaseswheretheexactsourceofpaincanbedetermined—suchasaherniateddisc,scoliosis,orspinalstenosis.

MINIMALLYINVASIVESURGERY • Traditionally,spinesurgeryisusuallyperformedasopensurgery.Thisentailsopeningtheoperativesitewithalongincisionsothesurgeoncanviewandaccessthespinalanatomy.However,technologyhasadvancedtothepointwheremorespineconditionscanbetreatedwithminimallyinvasivetech-niques. • Becauseminimallyinvasivespinesurgery(MISS),doesnotinvolvelonginci-sions,openmanipulationofthemusclesandtissuesurroundingthespineisavoided,therefore,leadingtoshorteroperativetime.Ingeneral,reducingintraoperative(duringsurgery)manipulationofsofttissuesresultsinlesspostoperativepainandafasterrecovery.
